SERVICE CHANGE REQUEST

This form is for making changes to existing services. If you need a new listserv, groupmail box, or website, please use the CREATE NEW SERVICE form.

Note that even if you have existing resources, your organization MUST BE REGISTERED for the current academic year in order for the change request to be fulfilled.

LISTSERV CHANGES

Changes to listservs are typically changes of ownership. If other changes are required, use the "Any Other Change" field to describe your needs.

Enter changes required for up to three listservs. If more than three require changing, please submit more forms as needed.
 











 

GROUPMAIL CHANGES

Changes to groupmail boxes are typically issues of granting or removing user access.  If other changes are required, use the "Any Other Change" field to describe your needs.

Enter changes for up to two groupmail boxes.  If more than two require changing, please submit more forms as needed.
 









 

URL CHANGES (For assistance with Web Resources, please refer to the RESOURCES FOR STUDENT WEBMASTERS page)

Changes to URLs are typically granting a new webmaster access to the URL. If a different change is required, please use the "Any Other Change" field to describe your needs.

If adding more than one webmaster, please submit additional forms.
